According to Epi Info HelpDesk:
"EPI INFO 6 AND 64-BIT VERSIONS OF WINDOWS
Epi 6, by default, will not run under Windows Vista or Windows 7 64-bit. This is because Vista 64 and Windows 7 64 only have support for 32-bit and 64-bit programs. Epi 6, on the other hand, is a 16-bit MS-DOS program.
However, many users have found DosBox (http://www.dosbox.com) allows them to run Epi 6 in spite of their OS not supporting 16-bit programs. If you want to get Epi 6 running on your Vista or Windows 7 machine, you may want to try DosBox."

Here are instructions from DosBox. Maybe Epi Info HelpDesk or someone can provide more information on how to install and run Epi Info 6.

You will need an MS-DOS emulator to run Epi Info 6 if you're using the 64-bit versions of Windows Vista or Windows 7, as those operating systems no longer include support for MS-DOS programs. Epi Info 6 usually works fine on the 32-bit versions.
Epi Info 6 can be downloaded from http://wwwn.cdc.gov/epiinfo/html/ei6_downloads.htm.
